This contract serves as verification of agreement between ALPHA TO OMEGA
WHOLISTIC THERAPEUTIC SERVICES (and/or Gyrlie Girlz), hereafter called
ATOWTS and

(hereafter called Host), for performance, demonstration and/or distribution of
legitimate spa services and/or products.
HOST agrees to:
1. Pay ATOWTS $______ fee; $50 non-refundable deposit due 1 month prior
to event, on ______. Payment due 2 weeks prior to event, on _______. Balance
due 1 week prior to event on _______. 10 guest minimum - 20 guest maximum.
Note: Additional fee applies for extra mileage locations and/or extra guests.
Cancellations must be written. Cancellations within 48 hours of event forfeits
$50 of balance paid.
2. Pay parking fees for banquet facilities, hotels, conference centers, etc.
3. Confirm guest list and service selections 2 weeks prior to event. Individual
services must be 50% paid at this time. No refunds for cancellations
within 24 hours.
4. Provide adequate working space, free from distractions and/or obstructions.
Space must be available 1 hour prior to start time for preparation.
5. Be prepared with adequate supplies.
6. Adhere to appointment schedule, and be considerate and respectful of the time.
No refunds for late arrivals or no-shows.
7. Accept responsibility for having guests complete liability waivers.
8. Provide own liability insurance.
9. Immediately report any unusual incidents and/or activities to ATOWTS.
ATOWTS agrees to:
1. Be prepared with adequate supplies (oils, lotions, linens, candles, music, etc).
2. Maintain sanitary supplies and an orderly work environment.
3. Execute all reasonable efforts to protect the modesty and privacy of clients.
4. Inform clients of contraindications as well as therapeutic benefits of services
and/or products.
5. Provide treatments at preset pricing schedule. No single service should exceed
agreed time limit.
Note: Each guest receives a gift card, for discounts on future purchases.
Host receives an additional gift.
*ATOWTS will provide no insurance; assume liability or responsibility for Host
and/or guests. ATOWTS will not assume responsibility for any damages caused by
services performed and/or products used. We reserve the right to refuse and/or
cancel services due to potential danger and/or posed risk of harm to our staff.
Prices subject to change without notice. I have read and understand the terms of
